Immunization Records
Welcome to the Humber Institute of Technology and Advanced Learning (Humber) and the University of Guelph-Humber! To help you with completing your immunization record and obtaining your clinical passport, the Health Centre has some suggestions for you.
Students in the following Health Science programs will need to follow the Paramed link below to find out about their Immunization Record and clinical passport procedure.

If you cannot see your doctor, you are able to come to the Health Centre for blood work and your shots.
For all other students in the Health Sciences programs and the School of Social and Community Services, the Health Centre requires the following:
- Proof of immunity (lab test results) for measles (red measles), rubella (German measles), mumps and varicella (chickenpox) is required. Please bring all copies of lab results with you to the health centre with the immunization record. The results of your blood tests may take 3-6 weeks. Please ensure you give yourself enough time to complete your immunization form before your clinical placement begins.
- A two step TB skin test. The second TB skin test must be given between 7 -21 days after the first injection. Please ensure you give yourself enough time to complete the two step TB test before your clinical placement begins. You will also require an annual one step TB skin test until your program is completed.
- Dates of your last Td shots. In order to receive your clinical passport you are required to have an updated Tetanus Diphtheria vaccine within the last ten years. If you are due for this shot it is recommended that you receive Adacel, a vaccine that protects you from tetanus, diphtheria, and pertussis (whooping cough). These vaccines are available at the Health Centre.
- Proof of primary series or date of last Polio booster is NOT mandatory. Routine polio immunization is not considered necessary for previously unimmunized adults in Canada who do not intend to travel to areas in which polio outbreaks occur, as there is a negligible risk of exposure to wild polioviruses in the Americas.
- Immunization with IPV is recommended for previously unimmunized adults and children who may be or are exposed to wild polioviruses. These include:
- Travellers to areas of countries where these viruses are circulating
- Residents of communities in which a visitor or new refugee/immigrant may be excreting the viruses
- Health care workers in close contact with individuals who may be excreting the viruses
- Laboratory workers handling specimens that may contain the viruses
- Hepatitis B. The Health Centre recommends that you protect yourself from hepatitis B infection through vaccination. Your future occupation and your clinical placements put you at risk for hepatitis B infection. All programs require proof of immunity for hepatitis B except the Humber ECE program. If you haven’t had the hepatitis B vaccine, the health centre offers it at a reduced cost. This involves getting 3 shots over 6 months. You will require 2 doses before receiving your clinical passport. Please ensure that you allow yourself at least 6 weeks to receive the first 2 doses.
- Influenza Vaccine (Flu). This is a requirement for nursing and paramedic students only. The health centre offers flu clinics in November of each year. We strongly recommend all students receive this vaccine since the flu virus can spread rapidly through the campuses. The flu is more serious than a cold and you can be sick for 3 to 5 days.
If you are not immune to hepatitis B, measles, rubella, mumps and varicella, you will require vaccination before obtaining your clinical passport. The chickenpox vaccine requires that you receive 2 doses one month apart. This could delay your ability to start your placement on time. We recommend that you come to the health centre during the summer to ensure that you meet all the necessary requirements. We can issue you a temporary passport after one dose of chickenpox.
If it is difficult for you to see your doctor to have your immunization record completed, the health centre can assist you with the following:
- Blood tests to check for immunity (no cost, valid Health insurance card is required)
- TB skin tests at a cost of $5.00 each
- MMR at a cost of $5.00
- Adacel at a cost of $5.00 for those 19 years of age, $32 for those over 19
- Hepatitis B vaccine at a cost of $25.00 each
- Varivax (Chickenpox) vaccine at a cost of $65.00 each
Paying for your shots:
You must pay for your Adacel, Hepatitis B, Gardasil, Menactra and Varivax vaccines at the Registrar’s office. Payment for all other vaccines is accepted in the health centre.
